Used during tenant screening. Part A is completed by the applicant to authorize us to contact prior landlords. Part B is completed by the current or former landlord to verify tenancy.

Applicant Authorization

To be completed by the applicant.

I hereby give my current/former landlord(s) permission to release any and all rental history information below including payment history, damages/repairs and other general info. Rental Depot and its authorized agents will use this information to approve or disapprove my application for residency.
Part B

Landlord Verification

To be completed by the current or former landlord. Please complete and email or fax the form back today.

1. Rent Payment

Is (was) applicant current on all rent?
Has he/she ever been late?
Have (had) you ever begun eviction?

2. Caring for the Unit

Does (did) applicant keep the unit clean?
Has (had) the applicant damaged the unit?
Has (had) the applicant paid for damages?
Will you (did you) keep the security deposit?

3. General

Does (did) the applicant permit persons other than those on the lease to live in the unit?
Does the applicant interfere with the rights and quiet enjoyment of other tenants?
Has the applicant given you any false information?
Have you fined or cited applicant for failure to follow HOA or apartment rules and regs?
Would you rent to this applicant again?
